MDS Assessment Coordinator (LPN/RN)
Location: Mount Carmel Senior Living Community, Mt. Carmel, PA
Mount Carmel Senior Living Community is seeking an experienced and detail-oriented MDS Assessment Coordinator (LPN/RN) to join our team. This critical role ensures accurate resident assessments and compliance with state regulations while supporting individualized care plans aligned with our facility’s standards.
Position Details
Schedule: Full-Time
Location: Skilled Nursing Facility, Mt. Carmel, PA
Key Responsibilities
Conduct resident assessments per schedule or as needed for condition changes and hospital returns.
Document and communicate condition changes accurately to appropriate personnel.
Collaborate with interdisciplinary teams to complete MDS, CAAs, and care plans, ensuring compliance and proper documentation.
Prepare and manage MDS schedules for the care team.
Transmit MDS data to the state and maintain accurate records, including transmittal logs.
Develop and regularly update care plans based on resident needs.
Qualifications
Active LPN or RN license in Pennsylvania.
Minimum 1 year of experience as an MDS Assessment Coordinator in a long-term care setting.
