*Requirements:*
* Minimum of 1+ year of *home health experience*.
* Proficiency with *OASIS* and *HCHB EMR documentation*.
* Current *MT Physical Therapist license (PT, DPT).*
*What You’ll Do:*
As a Home Health Physical Therapist, you’ll play a vital role in delivering care directly in patients’ homes. Responsibilities include:
* Performing patient assessments to evaluate functional status, mobility, and endurance.
* Collaborating with physicians to develop and implement individualized care plans.
* Administering treatments aimed at improving mobility, reducing pain, and restoring function.
* Documenting progress in alignment with *OASIS* and *HCHB EMR requirements*.
* Educating patients, families, and caregivers on care plans, exercises, and therapy goals.
