Responsible for managing and integrating Care Coordination services within a health system. Provides person-centered care, coordinates patient services, and collaborates with clinical leaders. Requirements include RN with BSN, New Mexico Nursing License, and 2 years of healthcare experience. Must have knowledge of healthcare facilities, strong communication skills, and ability to work with diverse populations. Reports to Manager of Population Health and Director of Ambulatory Patient Care Services. Works in various healthcare settings.
