Job Schedule: Full Time, 40 hours, Monday-Friday, with travel to Manchester and Bloomfield locations.

• Assess and address patients’ physical and psychosocial needs.
• Formulate and implement nursing care plans.
• Triages calls and initiates appropriate interventions.
• Maintain confidentiality and provide empathetic support to patients and families.

Qualifications: Current Connecticut RN License required; experience in a Medical/Surgical Office preferred; familiarity with medical software.
