Job Description
Seeking a PRN Licensed Certified Social Worker in Athens, TX (zip codes 75751 & 75752), with potential to become full-time.
Provide in-home individual and family counseling, complete forms, consult with providers, make referrals, monitor medication compliance, and develop support groups.
Perform hospital visits, case management, advocacy, and resource linkage.
Maintain professional relationships with staff and referral sources.
Requirements include a Master’s in social work (LCSW or BSW) and 1-3 years of hospice or home health experience preferred.
Must work independently and in groups, demonstrating professionalism.
