Job Description
The PHP Clinician conducts patient evaluations, assessments, and diagnoses, providing treatment recommendations.
Assists with developing, implementing, and adjusting treatment and continuing care plans.
Performs case management, crisis intervention, and discharge summaries, ensuring coordinated care.
Maintains compliance with payer requirements through chart reviews and documentation.
Participates in ongoing education, supervision, and community outreach activities.
Serves as a mentor to interns and students, upholding ethical standards and confidentiality.
Minimum of a Master’s degree, 1-2 years supervised counseling experience, and (preferred) independent licensure (LICSW, LMHC, or LMFT).
