We are seeking a dedicated and compassionate full time Occupational Therapist to join our team. This role involves developing personalized care plans, conducting assessments, and implementing therapeutic interventions to enhance patients’ functional abilities and overall well-being.
*Duties*
* Develop and implement individualized care plans based on patient assessments and goals.
* Conduct evaluations to determine patients’ physical, emotional, and social needs.
* Collaborate with interdisciplinary teams to ensure comprehensive patient care.
* Assist in discharge planning by preparing patients for a smooth transition from one care setting to another.
* Educate patients and their families on treatment plans and strategies for improving daily living skills.
* Maintain accurate documentation of patient progress and treatment outcomes.
*Qualifications*
* A degree in Occupational Therapy from an accredited program.
* Valid state licensure or certification as required by local regulations.
* Experience working in skilled nursing facilities or home health settings is preferred.
* Strong communication skills to effectively interact with patients, families, and healthcare teams.
* Experience using PCC and NetHealth preferred.
Job Type: Full-time
Pay: From $44.00 per hour
Expected hours: 32 – 40 per week
