Job Description

The RN Case Manager position in Brighton, MI involves planning, organizing, and directing hospice care with a focus on community health education/experience. Responsibilities include assessing patient and family needs, maintaining accurate clinical notes, and traveling throughout the day. Requirements include RN licensure, a clean driving record, valid driver’s license and auto insurance, and at least 1 year of nursing experience in acute care, long term care, or hospice/home health. Hospice experience is preferred.
