Heritage Health Care Center is hiring a Physical Therapist with a $10K Sign-On Bonus for full-time. The PT will provide direct rehab care to patients, following physician referrals and adhering to all laws and regulations. Requirements include a degree in physical therapy, state licensure, CPR certification, and at least one year of experience in post-acute care. Responsibilities involve creating treatment plans, setting goals, overseeing care by assistants, charting, utilizing therapy software, and ensuring patient safety.
