Provide direct rehab care in physical therapy per physician referral.
Must be a graduate of an accredited physical therapy program and hold a current state license.
One year of post-acute care experience preferred.
Participate in community health projects and maintain professional relationships.
Follow treatment plans, set goals, and oversee care provided by support staff.
Exhibit excellent customer service and maintain confidentiality.
